Samenvatting

Background
Overhydration in chronic renal insufficiency may lead to an increased risk of mortality. In dialysis patients bio-impedance analysis is used to determine volume status, whereas, in pre-dialysis patients volume status is mostly determined on „clinical assessment‟. In this study comparison between bio-impedance analysis and „clinical assessment‟ to determine overhydration in predialysis patients was made.
Methods
In 26 predialysis patients overhydration was independently determined by both „clinical assessment‟ (index test) and bio-impedance analysis (reference test).
Results
The cut-off point was >4 for „clinical assessment‟ with a sensitivity of 33%, a specificity of 73%, and a prevalence of 58%. The predictive values for a positive and negative test were respectively 62% and 44%. The positive and negative likelihood ratio was respectively 1, 22 and 0, 91. The AUC was 0, 55.
Conclusion
In predialysis patients it is not sufficient enough to determine overhydration only on „clinical assessment‟.
