Samenvatting

Samenvatting
Doel:
In opdracht van Revalidatiecentrum Blixembosch is een onderzoeksstudie uitgevoerd met als titel: "Analyse van de submaximale inspanningstesten". Het onderzoek betrof een literatuur studie naar de verschillende submaximale inspanningstesten.
De probleemstelling van de literatuurstudie luidt als volgt:
Het vaststellen van de meest betrouwbare en/of, valide en/of, bruikbare en/of, responsieve submaximale inspanningstest om de VO2max waarden te schatten bij patiënten met chronische a-specifieke lage rugklachten en oncologie patiënten na een chemokuur.
Tijdens het onderzoek is gebleken dat niet aan de onderzoekspopulatie van toepassing op Revalidatiecentrum Blixembosch voldaan kon worden. In plaats daarvan is een gezonde populatie gekozen.

Methode:
De literatuur is via een digitale databases gevonden en via in- en exclusie criteria geselecteerd, waarna de methodologisch kwaliteit van de geselecteerde literatuur beoordeeld werd aan de hand van een ontwikkelde methodologische lijst. (19) Hoofdpunten voor de beoordeling van de literatuur waren: betrouwbaarheid, validiteit, bruikbaarheid en responsiviteit. Dit zijn criteria die Revalidatiecentrum Blixembosch als eis aan een submaximaaltest stellen.
Na het toepassen van de methodologische lijst en in- en exclusiecriteria zijn er 16 artikelen overgebleven. Van deze artikelen over de volgende testen werd de data geëxtraheerd:
Canadian Aerobic Fitness Test (CAFT), Treadmill testen, Rockport fitness walking testen (RFWT), American College of Sports and Medicine testen (ACSM), Åstrand test, Submaximale fiets testen,
6-minute walk test, Extrapolatie test, 1-mile walk test.

Discussie:
Na de data-extractie zijn er een aantal discussiepunten ontstaan.
Vanwege het gebrek aan informatie over onderzoek naar submaximaal testen met patiënten die chronische a-specifieke lage rugklachten hebben of oncologie patiënten na een chemokuur, is gekozen voor een gezonde onderzoekspopulatie.
Er zijn veel onderzoeken naar submaximale inspanningstesten gedaan die in de jaren 60/ 70 hebben plaatsgevonden. Er is gekozen voor het zoeken in een digitale database naar literatuur, hierdoor zijn deze artikelen niet op deze manier te traceren. Hierop aansluitend kan men zich afvragen of onderzoeken ouder dan 30 jaar nog wel voldoen aan de standaarden/ eisen van deze tijd!
Revalidatiecentrum Blixembosch gebruikt de Åstrand submaximaal test als diagnostisch middel en daarbij is het meten van vooruitgang of achteruitgang essentieel! Er wordt in de gebruikte literatuur voor dit onderzoek weinig vermeld over test-hertest betrouwbaarheid en responsiviteit.
In de beoordeelde artikelen wordt weinig vermeld over de absolute 100% validiteit en betrouwbaarheid van de verschillende gouden standaarden. Dit is met name van belang voor de criteria gerelateerde validiteit en betrouwbaarheid van de submaximaal testen.
De gestandaardiseerde meetcondities worden praktisch niet toegepast in de verschillende protocollen/ methodes. Deze standaarden zijn van wezenlijk belang, willen de resultaten valide en betrouwbaar genoemd worden. (2,16,20,25) In de beoordeelde artikelen worden de protocollen zeer summier beschreven. Ze zijn niet aan een beoordeling onderworpen of al in een eerdere studie gebruikt. Het is dan ook opvallend dat er bij veel beoordeelde onderzoeken niet op betrouwbaarheid beoordeeld wordt. Voor de equaties die gebruikt worden geldt hetzelfde. De informatie die hierover geleverd wordt is over het algemeen erg summier en niet reproduceerbaar. Dit is een grof minpunt van de verschillende onderzoeken, omdat de equatie de basis is voor het onderzoeksresultaat.
De volgende algemene aanbeveling kan ook aan Revalidatiecentrum Blixembosch gedaan worden: Er wordt in McArdle et al., (1996) geadviseerd dat gedurende training voor specifieke aërobe activiteiten zoals fietsen, zwemmen, roeien of rennen het testmiddel ook in deze activiteiten plaats zal moeten vinden. Dit is voor de toekomstige keuze voor een trainingsvorm in combinatie met een testmiddel belangrijk.


Aangezien er onvoldoende literatuur over de Åstrand test is gevonden en beoordeeld kan deze literatuurstudie het gebruik van de Åstrand submaximaal test in Revalidatiecentrum Blixembosch niet valideren. Het lijkt dan ook dat de Åstrand test gebruikt wordt aan de hand van een recent niet onderbouwde reputatie. Dit geldt overigens niet alleen voor de Åstrand test, maar ook voor alle andere submaximaaltesten. Deze literatuurstudie kan het gebruik van de submaximaaltest als referentiewaarde voor een maat van de aërobe conditie niet valideren.

Conclusie:
Er is geen enkele test uit het onderzoek gevonden die voldoende betrouwbaar, valide, bruikbaar en responsief scoort om te kunnen reproduceren naar Revalidatiecentrum Blixembosch.

Abstract
Objective
The rehabilitation centre Blixembosch was the assignor of a graduation project review study with the following title:
"Analysis of submaximal endurance tests'. The research enhanced a literature study.
The following aim was developed:
Determine the most reliable and/or, valid and/or, responsive and/or, useful submaximal test to predict VO2max in a population of patients with chronic low back pain and cancerpatients after chemotherapy.
In the research there has not been found any information about a population of patients with chronic low back pain and cancerpatients after chemotherapy, therefore a healty population has been used.

Design
A digital database has been used to find the literature. In-and exclusion criteria have been used and after that the methodological quality of the selected literature was judged through the developed methodological list. (19) The most important points of the judgement were: reliability, validity, usefulness, and responsiveness. Those criteria were determined to be important for a submaximal test in rehabilitation centre Blixembosch.
From the following submaximal tests a summary was extracted:
Canadian Aerobic Fitness Test (CAFT), Treadmill tests, Rockport fitness walking tests (RFWT), American College of Sports and Medicine tests (ACSM), Åstrand test, Submaximal cycling tests,
6-minute walk test, Extrapolatie test, 1-mile walk test.

Discussion
After the extraction of the data there were some discussion points. The population in the current study doesn't correspond with the population at the rehabilitation centre Blixembosch. There are many studies that were developed in the 60's and 70's. The choice was made to search in databases on the internet, hence these articles were not included. It's reasonable to question the usefulness of articles older than 30 years. Can they respond to the latest standards/ demands of this time.
Rehabilitation centre Blixembosch uses the Åstrand submaximal test for diagnostic means and in a diagnostic point of view it's very important to measure progression or regression in a persons aerobic capacity. Strangely enough in most of the articles nothing much is said about the test- retest or responsiveness. In the articles nothing much is said about the 100% validity and reliability of the so-called gold standard is mentioned. This is extremely important for the reproducibility of the criteria related numbers. The standard measure conditions have not been followed in most of the researches. These standards are very important, to determine whether a test is valid and reliable.
In the judged articles, the methods/ protocols are described very shortly and there is a lack of information about these items. Most of the time they have not been used in previous studies. Thus, it's very strange that no study is done on the reliability .The same problem is accountable for the used equations. The lack of information in the used equations makes it impossible to reproduce to the assignee. The equation is the base for the VO2max value, thus it's very important.
The following general advice can be given to our assignor. McArdle et al. advised that the training and submaximal test should be done in the same test discipline. This is very important for the future choice of a submaximal test and aerobic training methods, for example the choice between a bike or treadmill. Because not much literature has been found about the Åstrand submaximal test, the use of the Åstrand test cannot be validated. It seems that the use of the Åstrand submaximal test depends on his status rather than on its quality. This is not only the case for the Åstrand test, but for all submaximal aerobic tests investigated. This study cannot validate the use of a submaximal test as a reference for the measure of a persons aerobic endurance capacity (VO2max).

Conclusion
Not one single test was found to be reliable, valid, responsive or useful to reproduce to rehabilitation centre Blixembosch.
