Samenvatting

Inleiding
Dit onderzoeksverslag is geschreven in het kader van de afstudeeropdracht in het vierde studiejaar van de Opleiding voor Fysiotherapie aan de Fontys Paramedische Hogeschool te Eindhoven. Het projectvoorstel is ingediend door Revalidatiecentrum Blixembosch te Eindhoven, afdeling fysiotherapie.

Er zijn drie onderzoeksvragen geformuleerd:

1. Hoe ziet een evidence based oefenprogramma er uit, gebaseerd op de meest recente literatuur (vanaf 2003 tot heden), de KNGF richtlijn 'Beroerte'1 en de 'Ergotherapierichtlijn Beroerte'4, voor respectievelijk de zit-, sta- en loopgroep van CVA-revalidanten tijdens de subacute en postacute fase?

2. Aan welk niveau of score van een meetinstrument moet een CVA-revalidant voldoen, gerelateerd aan evidence based meetinstrumenten voor het meten van in- en uitstroomcriteria5 van respectievelijk de zit-, sta- en loopgroep, om in één van deze groepen deel te mogen nemen?

3. Wat is de meest effectieve en efficiënte manier om de communicatie tussen de verschillende fysio- en/of ergotherapeuten van respectievelijk de zit-, sta- en loopgroep te laten plaatsvinden?

Het doel van het project is het ontwikkelen van een oefenprogramma, samengebracht in een klapper.

Methode
Voor het realiseren van het hoofddoel is een literatuurstudie uitgevoerd. Hiervan zijn uiteindelijk 21 artikelen gebruikt, 11 artikelen zijn relevant bevonden voor de zitgroep, 12 artikelen zijn relevant voor de stagroep en 19 artikelen zijn van belang voor de loopgroep.
Voor het vinden van evidence-based meetinstrumenten, is gebruik gemaakt van het boek 'Klinimetrie na een beroerte: een praktische handleiding'13. Er zijn 5 meetinstrumenten geselecteerd voor toetsing van in- en uitstroomcriteria voor respectievelijk de zit-, sta- en loopgroep.
Voor de mogelijke manieren waarop communicatie over respectievelijk de zit-, sta- en loopgroep plaats kan vinden is de KNGF richtlijn 'Verslaglegging' geraadpleegd en gebrainstormd.

Resultaten
Uit de gevonden artikelen zijn 26 specifieke verwijzingen naar oefeningen en aanbevelingen gehaald die in het oefenprogramma verwerkt zijn. Een vaak voorkomende aanbeveling is taakspecifiek oefenen. Het oefenprogramma is dan ook grotendeels hierop gebaseerd.
Voor het beoordelen of een revalidant in aanmerking komt voor de zitgroep zijn de Trunk Control Test en Berg Balance Schaal van belang. Voor de stagroep zijn dit de Berg Balance Schaal en Timed Balance Test. Voor de loopgroep zijn de Functional Ambulation Categories en 10-meter looptest van belang.
Er is een communicatievel ontwikkeld, bedoeld voor fysio- en ergotherapeuten die de groepen leiden, waarop bijzonderheden over de gegeven lessen en revalidanten genoteerd kunnen worden.

Conclusie
Er is een geplastificeerde klapper ontwikkeld bestaande uit een up-to-date en evidence based oefenprogramma voor respectievelijk de zit-, sta- en loopgroep voor CVA-revalidanten in revalidatiecentrum Blixembosch tijdens de subacute en postacute fase, een advies voor vijf evidence based meetinstrumenten en een communicatievel.


Summary

Purpose of the project
This research report is the final thesis in the fourth study year of the Physiotherapy course at Fontys Paramedic College in Eindhoven. The subject of this project was requested by the Physiotherapy department in rehabilitation centre Blixembosch in Eindhoven.

The main research questions were:

1. What does an evidence-based exercise program for the sit, stand and walk groups of stroke rehabilitants during the sub-acute and post-acute phase after a stroke look like? This program is based on the most recent literature (from 2003 until the present), the KNGF richtlijn 'Beroerte' (Royal Dutch Physiotherapy Society Stroke Guideline) and the Ergotherapierichtlijn 'Beroerte' (Occupational Therapy Stroke Guideline).

2. Which level or particular values must a stroke-rehabilitant achieve, related to evidence-based measurement tools for measuring the in- and exclusion criteria for the sit, stand and walk group, to be allowed to take part in one of these groups?

3. What is the most effective and efficient method of communication between the different physio- and occupational therapists of the sit, stand and walk groups?

The project's goal is to develop an exercise program, which is compiled in a folder.

Method
To realize the main goal, relevant literature was studied. From these articles, 21 articles were eventually used, 11 articles were found relevant to the sit group, 12 articles were found relevant to the stand group and 19 articles were found relevant to the walk group.
To find evidence based measurement tools, the book 'Klinimetrie na een beroerte: een praktische handleiding'13 was used. Five evidence based measurement tools were selected to determine if a rehabilitant was able to take part in the sit-, stand- or walkgroup. The KNGF richtlijn 'Verslaglegging' was consulted and then a brainstorm session followed about the possible methods upon which the communication for the sit-, stand- and walkgroup could take place.

Results
In the literature 26 specific references were found for exercise material or exercises. A great deal of literature referred to task specific exercising. The exercise program is largely based on this.
To determine is a rehabilitant is able to take part in the sitgroup, the Trunk Control Test and the Berg Balance Scale are important. For the standgroup the Berg Balance Scale and the Timed Balance Test are important and for the walkgroup the important tests are the Functionale Ambulation Categories and the 10-meter walktest.
A communication sheet was developed, meant for the physical- and occupational therapists that lead the groups, upon which they can write information about the lessons or rehabilitants.

Conclusion
A plastified folder has been developed containing an up-to-date and evidence based exercise program for the sit-, stand- and walkgroup for stroke rehabilitants in the rehabilitation centre Blixembosch during the sub-acute and post-acute phase, five advised evidence based measurement tools and a communication sheet.
