Samenvatting

Doel:
Het doel van dit onderzoek was het ontwikkelen van een richtlijn, inclusief handleiding, met een goed onderbouwd oefenprogramma inclusief metingen en beoordelingsformulieren hiervoor. Deze is bestemd voor de afdeling fysiotherapie van het Catharina-ziekenhuis te Eindhoven waar patiënten na een voorste kruisbandreconstructie of een total knee-operatie behandeld worden in poliklinische trainingsgroepen.

Methode:
Er is gestart met het zoeken naar literatuur betreffende de voorste kruisbandreconstructie en total knee-operatie. De gevonden literatuur is geselecteerd en toegepast in het theoretische kader. Daarna is wetenschappelijke literatuur gezocht over oefentherapie bij patiënten na een voorste kruisbandreconstructie of total knee-operatie in de postklinische revalidatieperiode. Vervolgens volgde een onderzoek naar protocollen die hiervoor in de praktijk toegepast worden. Tevens is onderzoek gedaan naar de verschillende meetinstrumenten bij beide patiëntencategorieën pre- en postoperatief. De gevonden resultaten uit de geselecteerde artikelen zijn naast elkaar gelegd en vergeleken. De meest uitgesproken, overeenkomstige resultaten uit de geselecteerde artikelen zijn samengevat en beschreven in het verslag.
Vervolgens volgde een vergelijking van de protocollen, de wetenschappelijke literatuur en de groepstherapie zoals nu in het Catharina-ziekenhuis gegeven wordt aan patiënten na een voorste kruisbandreconstructie of een total knee-operatie.

Resultaten:
In de resultaten staan de verschillende vormen van oefentherapie in de postklinische fase voor patiënten na een voorste kruisbandreconstructie of een total knee-operatie die gevonden zijn in de wetenschappelijke literatuur en de protocollen. Hierin is naar voren gekomen dat er veel verschillende inzichten zijn hierover.
Ook staan in de resultaten de verschillende meetinstrumenten voor beide patiëntencategorieën vermeld die zijn gevonden na onderzoek in de wetenschappelijke literatuur. Deze meetinstrumenten zijn vergeleken met betrekking tot validiteit, betrouwbaarheid, praktische toepasbaarheid, prijs en tijdsduur.

Conclusie:
Op basis van het onderzoek is gebleken dat de bestaande groepstherapie in het Catharina-ziekenhuis in mindere mate evidence based en best practise is. Deze is door de onderzoeksgroep aangepast.
De volgende meetinstrumenten worden aanbevolen in de voorste kruisbandgroepen: VAS-score, Biodex, Goniometer, Patellamobiliteit en patellasporing, KT-2000 Arthrometer, Single-leg Hop Test, Vertical Jump Test, Lysholm Score, Tegner Score, Anterior Cruciate Ligament Quality of Life Questionnaire en EKAA-Score.
De volgende meetinstrumenten worden aanbevolen in de total knee-groepen: Goniometer, VAS-score, Patellamobiliteit en patellasporing, Timed Up & Go, 6 Minute Walk Test, Timed Stair Test, Oxford 12-item Knee Score en EKAA-Score.
Bovenstaande is samengevat in een richtlijn. Deze bestaat uit een goed onderbouwd oefenprogramma inclusief metingen, beoordelingsformulieren en een handleiding.








Summary


Objective:
The purpose of this research was to develop a guideline, including a manual, with a well found treatment program including measurements and appraisal forms. This guideline is designed for the department physical therapy of the Catharina-ziekenhuis in Eindhoven where patients are being treated in outpatient training groups after an anterior cruciate ligament reconstruction or total knee surgery.

Design:
A literature study is started in relation to anterior cruciate ligament reconstruction and total knee surgery. The literature found is selected and applied into a theoretical frame.
After that, scientific literature about physical therapy with patients after an ACL-reconstruction or total knee surgery in the post-clinical rehabilitation phase was searched.
Then a study into protocols followed which are applied for this in practice.
Also study into the different measurement instruments has been done at both patient categories pre- and postoperative.
The found results from the selected articles have been compared. The most pronounced, corresponding results from the selected articles have been summarized and have been described in the report.
After that a comparison of the protocols, the found scientific literature and the group lessons given patients after first ACL-reconstruction or total knee surgery in the Catharina-ziekenhuis followed.

Results:
In the results the different forms of remedial therapy in the post-clinical rehabilitation phase for patients after first ACL-reconstruction or total knee surgery are described which are found in scientific literature and the protocols. There are many different views about these subjects.
Also mentioned in the results are the different measurement instruments for both patient categories which have been found after research in scientific literature. These measurement instruments are compared with regard to validity, reliability, practical applicability, costs and duration.

Conclusions:
On account of the research it appeared that the existing group therapy in the Catharina-ziekenhuis is less evidence based and best practice. This has been adapted by the research group.
The following measurement instruments are recommended in the ACL-reconstruction groups:
VAS-score, Biodex, Goniometer, Mobility of the patella and lining of the patella, KT-2000 Arthrometer, Single-leg Hop Test, Vertical jump test, Lysholm Score, Tegner Score, Anterior Cruciate Ligament Quality of Life Questionnaire and EKAA-Score.
The following measurement instruments are recommended in the total knee surgery group:
Goniometer, VAS-score, Mobility of the patella and lining of the patella, Timed Up & Go, 6 Minute Walk Test, Timed Stair Test, Oxford 12-item Knee Score and EKAA-Score.
All of the above has been summarized in a guideline. This guideline consist of a well found treatment program including measurements, appraisal forms and a manual.
