Samenvatting

Doel:
De doelstelling van dit onderzoek is een overzicht te krijgen van de stand van zaken betreffende de communicatie tussen fysiotherapeuten en huisartsen in de regio Eindhoven, voor- en na invoering van Directe toegankelijkheid Fysiotherapie (DTF)

De volgende vraagstelling wordt beantwoord:
Is de communicatie tussen huisartsen en fysiotherapeuten in de regio Eindhoven veranderd na invoering van Directe Toegankelijkheid Fysiotherapie in 2006 en wat zijn deze veranderingen?

Methode:
Het onderzoek was tweeledig. Ten eerste is er een literatuuronderzoek uitgevoerd om een theoretisch kader te vormen en ideeën te verkrijgen voor het formuleren van de enquêtevragen.
Ten tweede is er een praktisch onderzoek uitgevoerd in de regio Eindhoven om de verandering in communicatie tussen huisartsen en fysiotherapeuten in kaart te brengen.
In totaal zijn er 16 huisartsenenquêtes en 42 fysiotherapeutenenquêtes geanalyseerd.

Resultaten:
Van de huisartsen die participeerden in het onderzoek geven 7 personen (43,75%) te kennen dat er minder vaak contact met fysiotherapeuten is over patiënten die via DTF bij de fysiotherapeut komen. Dit in vergelijking met patiënten die op verwijzing bij de fysiotherapeut komen. 23 fysiotherapeuten (54,7%) geven aan dat dit contact hetzelfde is gebleven.
Op het gebied van verwijzing, verslaglegging, overleg en verwachtingen is de communicatie vóór, in vergelijking met ná de invoering van DTF, vrijwel onveranderd gebleven.
Niet alle fysiotherapeuten hebben knelpunten in communicatie met de huisarts aangegeven. Van de 12 fysiotherapeuten die dit wel hebben gedaan geven er 4 (33,3%) aan dat de huisartsen geen verslag van de screening willen ontvangen.
Van de geënquêteerde huisartsen geven 15 personen (93,75%) aan altijd een eindverslag te willen ontvangen, het grootste deel van hen wil dit in schriftelijke of digitale vorm. Daarnaast vinden 10 huisartsen (62,5%) dat de eindverslagen van de fysiotherapeuten voldoen aan de eisen die zij als huisarts eraan stellen. Als er gedurende de behandeling contact nodig is, dan willen 11 huisartsen (68,75%) dat dit telefonisch plaatsvindt.
Het verslag over de screening willen de meeste huisartsen alleen maar bij een 'niet pluis' gevoel van de fysiotherapeuten ontvangen. 12 huisartsen (75%) willen benaderd worden als de fysiotherapeut een rode vlag ontdekt bij een patiënt, van hen willen 7 huisartsen (43,75%) diezelfde dag benaderd worden, en 3 (18,75%) direct na screening.
De ontvangst van informatie van de screening in schriftelijke vorm is gewenst door 8 huisartsen (50%), 5 huisartsen (31,3%) willen die informatie digitaal ontvangen en 3 huisartsen (18,8%) willen het via de telefonische weg wil.

Door de auteurs van het onderhavige project is de hypothese gesteld dat er relaties bestaan tussen de demografische gegevens van de huisartsen en fysiotherapeuten en de communicatie tussen beide. Dit is gebaseerd op bevindingen uit de literatuurstudie, waarin onder andere is gevonden dat een verschil in perspectief bestaat bij het voeren van een voorlichtingsgesprek tussen fysiotherapeuten die biomedisch- of psychosociaal georiënteerd zijn. Hierover kunnen vanuit de gegevens van het onderzoek geen significante uitspraken worden gedaan.



Conclusie:
Geconcludeerd kan worden dat de er in de communicatie tussen huisartsen en fysiotherapeuten in de regio Eindhoven nauwelijks iets veranderd is. Dit betreft de frequentie van communicatie en de wijze waarop deze plaatsvindt.
De Directe Toegankelijkheid Fysiotherapie heeft de communicatie tussen fysiotherapeuten en huisartsen niet beïnvloed.

Aanbevelingen
Er is weinig veranderd in de communicatie tussen huisarts en fysiotherapeut vóór en ná invoering van DTF. Het is echter niet duidelijk of de communicatie goed was en dat nu nog is, of dat de communicatie slecht was en dat nu nog steeds is. Het verdient de aanbeveling dit nader te onderzoeken.

Er is een enquête afgenomen bij huisartsen en fysiotherapeuten, het waarom bij de beantwoording van de vragen is echter niet bekeken. Het belichten van deze achterliggende gedachte is een aanbeveling voor navolgend onderzoek.




Summary

Objective
The goal of this study is to obtain an overview of the situation concerning the communication between physiotherapists and general practitioners in the region of Eindhoven before and after the introduction of Direct Accessibility of Physical therapy (DTF).

The following definition of a problem will be answered:
Has the communication between general practitioners and physiotherapists in the region of Eindhoven changed after the introduction of Direct Accessibility of Physical therapy (DTF) in 2006 and if so what are these changes?

Design (method)
The study consisted of two parts. First a literature study was done to form a theoretical background and to obtain ideas for the questionnaire. Secondly a practical analysis is held in the region of Eindhoven to obtain a survey of the changes in the communication between general practitioners and physiotherapists. In total 16 general practitioners (GP) and 42 physiotherapists were interviewed.

Results
7 of the participating GP's (46,3%) report that there is a decrease in contact with physiotherapists concerning patients who come within DTF compared to the situation in which the GP's refer the patients. 23 of the participating physiotherapists (56,1%) report that this contact stayed the same.
Relating to referral, reporting, consultation and expectations the communication has undergone little changes when comparing the situation before and after the introduction of DTF. Not all physiotherapists reported bottle neck issues in the communication with GP's. Of the 12 therapists who did, 4(33%) reported that a problem was that GP's didn't want to receive a report of the screening. 15 of the interviewed practitioners (93,8%) report that they always want to receive the end report. The vast majority of this group says that they want to have that report by letter or digital. Next to that, 10 of the practitioners (62,5%) have the opinion that the end reports made by physiotherapists live up to their standards. If a therapist needs to contact a GP, it should be done by telephone according to 11 of the GP's (68,8%). The majority of the GP's is only interested in the screening report when a physiotherapist has his doubts about the complaint of a patient. 12 GP's (75%) want to be contacted in case of a red flag, 7 of them (43,75%) want to be contacted that same day, while 3 (18.75%) want it immediately after the screening.
In the literature study was found that there are relations between the demographic facts of GP's and physiotherapist and the communication between them. In the light of this study no further significant comments can be made on this subject.

The authors came up with the hypothesis that relations exist between the demographic information of the GP's and physical therapists, and the communication between them. This is based on findings in the literature study. For example, it is found that a difference in perspective exists during a consultancy in physical therapists with a biomedical- or psychosocial orientation. But based on the findings no significant statements can be made.

Conclusions
The following conclusion can be made: the communication between general practitioners and physiotherapist in the region of Eindhoven barely changed regarding the frequency of communication and the method in which it occurs. The Direct Accessibility of Physical therapy didn't had any influence on the communication between general practitioners and physiotherapists.


Recommendations

Almost nothing changed in the communication between general practitioners and physical therapists before and after the introduction of Direct Accessibility of Physical therapy.
However it is not clear whether the communication was good and still is good, or if the communication was and still is poorly. It deserves recommendation to investigate this further.

General practitioners and physical therapists filled in a questionnaire. The reason why they filled in a particular answers isn't made clear. This can be an interesting subject for further investigation.
