Samenvatting

Title
The influence of postural control exercises on re-learning gait in patients diagnosed with hemiplegia as a result of cortical CVA.

Main Question
What is the influence of postural control exercises on re-learning gait in patients diagnosed with hemiplegia as a result of cortical CVA?

Objective
To have knowledge about the influence of postural control exercises on re-learning gait in patients diagnosed with hemiplegia as a result of cortical CVA. In addition, to have a manual about the progression stages of the postural control exercises in relearning gait of hemiplegic patients secondary to cortical CVA.

Method
A systematic review and a literature study were performed. English search engines were used and resource persons were contacted. The methodological qualities of the articles were assessed using the PEDro and the Cochrane scales. A data extraction list was created to extract all the necessary information for analysis and synthesis. The manual was produced to serve as a simplified but a rich additional learning aid to students, especially English Stream Physiotherapy Students to compliment the limited text books available on the topic of review. It was developed through careful internet search, facts from other text books, previous knowledge and experience on the subject and last but not the least information from resource persons based on this systematic review.

Results
The results were worked out under the following headlines: Study selection, type of study, participant, intervention and the outcome.

Relevant articles were selected out of the total of 47 articles found, using the set inclusion criteria. 4 Articles passed the methodological quality test, and were included in this review. All the relevant data were extracted and presented for analysis and synthesis.

The heterogeneous outcome measures from the various articles used was scaled down to fit the purpose of this review and the outcome was interpreted accordingly. The result from this review showed that only one (Wang et el 2005) out of the 4 studies included in this systematic review, stated that Bobath/NDT treatment demonstrated significant improvements. These improvements were found in the following outcome measures: Stroke Impairment Assessment Scale (SIAS), motor control of lower extremity (p < 0.01), Motor Assessment Scale (MAS) (p < 0.001), Berg Balance Scale (BBS) (p ≤ 0.001) and in Stroke Impact Scale (SIS) (p = 0.005). Although Bobath/NDT deals with postural control exercises, it is just one aspect of the whole Bobath/NDT treatment technique. It was therefore difficult to interpret the result that postural control exercises has influence or not on re-learning of gait in patients diagnosed with hemiplegia as a result of cortical CVA.

Conclusion
Although there is sufficient evidence in literature that postural control exercises is very important when relearning gait in hemiplegic patients, it can not be concluded that postural control exercises has influence or not when relearning gait in patients diagnosed with hemiplegia secondary to cortical CVA since there was no concrete proof from this systematic review.
